What Is a Fax Cover Sheet?
A fax cover sheet is an introductory page placed before the documents you are sending. It tells the receiving office who should receive the fax, who sent it, how many pages to expect, and whether any action is requested.
Keep the cover message brief. Use it for routing information and instructions; place the full letter, report, or other document behind it. If you need a branded letter to accompany the fax, our editable letterhead template provides a separate starting layout.
What to Put on a Fax Cover Sheet
| Field | What to enter |
|---|---|
| Company name | Your organization’s name, replacing the sample text |
| To | The intended recipient’s name and, where useful, department or organization |
| From | The sender’s name and department or organization |
| Fax | The destination fax number, including the appropriate dialing codes |
| Phone | A callback number where the receiving office can reach the sender |
| Date | The sending date; spell out the month if a numeric date might be ambiguous |
| Pages | The total transmitted page count, with a note stating whether it includes the cover |
| Subject | A short description or reference that helps the recipient identify the documents |
| Message | Brief instructions, the requested next step, and a reply deadline when needed |
The template’s original “PHONES” label is ambiguous. Rename it “SENDER PHONE” if you want to make the purpose clear. Add a separate return fax number field if replies should come back by fax.
How to Customize the Word Template
- Open and save a copy. Keep the original download available and create a separate working document.
- Replace the company name. Enter your organization’s name in the box. Adjust the font size if needed to keep the name within the border.
- Correct and clarify the labels. Fix “SUBJETC,” use singular date and phone labels, and identify whose telephone number you are providing.
- Enter the fax details. Fill in the recipient, sender, destination number, date, subject, and message. Depending on the element, you may need to select a text box or position text beside the existing line.
- Set the page count last. Count the final packet after adding all attachments, and state your counting convention.
- Review the layout. Check Print Preview for clipped text, unexpected page breaks, and a message that extends beyond its box.
- Prepare the final copy. Print for a fax machine, or export to PDF if that format is accepted by your online fax service.
For consistent stationery across your outgoing documents, compare the company heading with the sample letterhead template and use the same organization name and contact details.
Does the Page Count Include the Fax Cover Sheet?
A clear approach is to count every page being transmitted and write “5 pages, including this cover sheet.” For example, a four-page attachment plus a one-page cover makes a five-page fax.
If your organization counts only the attachments, say so explicitly: “4 document pages, plus this cover sheet.” When an online fax service generates its own cover page, check the assembled transmission so you do not accidentally add a second cover or use the wrong count.
Sample Fax Cover Message
Replace the bracketed details with information for your transmission:
Please forward the attached [document name] to [recipient or department]. This transmission contains [number] pages, including this cover sheet. Please confirm receipt by [date or time] using [callback number or reply method]. If any pages are missing or unreadable, contact [sender name] at [phone number].
Keep sensitive details out of the subject and cover message when a general reference will do. The receiving office may see the cover page before it reaches the named recipient.
Check These Details Before Sending
- Verify the destination number against a trusted contact record rather than relying on an old cover sheet.
- Check the recipient’s name and department so the receiving office can route the packet.
- Put the cover first and confirm the order and orientation of the attached pages.
- Match the page count to the final transmission, including any service-generated cover.
- Check readability using a print or PDF preview; small text and faint graphics may reproduce poorly.
- Review the transmission result after sending. A successful transmission report does not establish that the intended person has read the documents.
Frequently Asked Questions
Is this a fillable PDF fax cover sheet?
No. The download is a Word document, not a PDF with interactive form fields. Edit it in Word and export a PDF copy if needed.
Can I print a blank cover sheet and complete it by hand?
Yes. Correct the sample labels, add your company details, and print blank copies. Write clearly in dark ink and keep the message inside the available space.
Can I use the template with an online fax service?
You can prepare a cover page from the template and upload it if your service accepts the resulting file format. Check whether the service adds its own cover page before submitting the fax.
Does it include a confidentiality notice?
The original design does not include a confidentiality notice. If your organization requires one, add its approved wording and keep it readable. A notice does not prevent a fax from being sent to the wrong number or control who views the received pages.
Can I use it for medical, legal, or financial documents?
This is a general office layout. Use the cover wording and sending procedures approved by your organization for the documents involved. The template itself does not establish compliance with industry or legal requirements.
