Record customer orders taken by phone with these six telephone sales order forms for Word. Choose a detailed order layout or a simpler item-and-delivery record, then adapt it to your products and fulfillment process.
Each download is a manually completed document. The forms do not calculate totals, process payments, check inventory, or send orders automatically.
Choose a Telephone Order Form
Format: all six downloads are Microsoft Word .docx files, downloaded directly rather than in ZIP packages.
Replace sample business details, clarify quantity and pricing labels, and review the terms before use. Forms 02 and 04 contain card-detail fields that should be replaced with a payment-status or processor-reference field in the ordinary order record.

What Is a Telephone Sales Order Form?
A telephone sales order form records what a customer requests during a call: who is ordering, what they want, quantities, prices, delivery details, and any agreed next steps. The seller uses the record to confirm and process the order.
It is different from a purchase order issued by the buyer, an invoice requesting payment, or a receipt acknowledging payment. A customer PO number can link those records without making them interchangeable.
Information to Confirm During the Call
| Detail | What to check |
|---|---|
| Customer and caller | Name, organization where relevant, callback number, and the caller’s role |
| Billing and delivery | Whether the billing party and delivery recipient differ, including complete addresses |
| Items | Item code, variant, description, quantity, and unit of measure |
| Price | Unit price, currency, discounts, and what the quoted amount includes |
| Additional charges | Applicable tax and delivery or handling amounts |
| Timing | Requested dates versus confirmed or promised dates |
| Payment | Agreed method, status, and approved payment reference |
| Follow-up | Order reference, confirmation method, and any outstanding checks |
Record an unknown amount or unconfirmed date explicitly. Do not turn an estimate into a promise just because a field is available.
How to Use a Phone Order Form
- Prepare your master. Add company details, define field meanings, and remove irrelevant sample wording.
- Identify the order. Assign an order reference and record the date and person taking the call.
- Capture the customer’s request. Confirm product codes, options, quantities, and delivery instructions.
- Check pricing and availability. Use current approved information and flag any item awaiting confirmation.
- Calculate and review amounts. These Word forms do not perform the arithmetic for you.
- Read back the key details. Confirm the items, quantities, address, price scope, and agreed timing with the caller.
- Record the next step. Note payment status, approval or availability checks, and how confirmation will be sent.
- Route and retain the record. Keep a controlled copy and pass fulfillment details to the appropriate team.
Check Line Totals and Discounts
A simple example is 4 items at 25 each: the line amount is 100. If an agreed order discount is 10 and delivery is 8, the subtotal after those adjustments is 98 before any applicable tax.
That example is arithmetic, not a tax rule. Define whether discounts are amounts or percentages, identify the tax basis separately, and avoid applying the same discount both on a line and at order level. Review form 04 particularly carefully because it has both line and summary discount fields.
Keep Payment Handling Separate
Forms 02 and 04 include full card-number and expiry fields. Use your approved payment process instead of storing card details in an ordinary Word order form or sending them with fulfillment instructions.
Replace those fields with entries such as Payment Method, Payment Status, and Processor Transaction Reference. Do not record a payment as received merely because a customer has agreed to pay. If the order is on account, the accounts receivable ledger offers a separate customer-account layout.
Connect the Order to Fulfillment and Billing
Use the shipping order template for a separate dispatch record when required. Carry the sales-order reference into the shipping document so the team can match what was ordered and sent.
The store inventory sheet can organize stock information, but entering a phone order does not update that workbook automatically. Confirm availability through the actual inventory process.
Browse the invoice templates for separate billing layouts. Keep order, invoice, payment, and shipment references connected rather than treating one document as proof of every stage.
Customization and Printing Checks
- Replace sample company names, addresses, phone numbers, and payment wording.
- Clarify Sold To, Bill To, Ship To, and Ship From where those labels appear.
- Review specialist terms such as F.O.B. and purchasing-agent authorization rather than adopting them without context.
- Add currency, item units, line totals, or approval fields where the chosen form lacks them.
- Check table widths and page breaks after adding items, then retain a checked final copy.
