A shipping order template helps you record sender and recipient details alongside the items associated with a shipment. Clear descriptions, quantities, and order references make the document easier for staff and recipients to check.
The free Word form below includes Shipping From and Shipping To sections, an order number and date, an item table, amount summaries, signature areas, and editable terms. Review all sample information before using it for an actual order.
Download format: ZIP archive containing one Microsoft Word document (.docx).
Included: Sender and recipient details, order reference, item descriptions, prices, quantities, totals, signature areas, and terms placeholders.

Extract the ZIP and open the document in Microsoft Word. Save a working copy. A separate Excel workbook or PDF is not included. Review our Terms & Conditions for usage information.
The document is an editable form, not a carrier booking tool. It does not generate a tracking number, purchase postage, update inventory, or connect to an order-management system. Do not assume that displayed totals calculate automatically; verify the arithmetic when completing the form.
Replace the logo placeholder, sample order number, and sample date. Use a reference that connects the document to your own order records. Keep dates unambiguous, especially when sharing the form across regions.
If the original order is taken by phone, a telephone sales order form can capture that request. Carry the same reference forward so the request and shipment remain connected.
Enter the sender’s name and address under Shipping From and the delivery recipient under Shipping To. Check street details, unit numbers, locality, postal code, and country where relevant.
The shipping address may differ from the billing address. Add a contact field or additional delivery instructions if your process needs them; those details are not all provided as dedicated fields in the original form.
Give each line a clear item description. Include a product code, size, color, or unit where necessary to identify what is being supplied. State whether a quantity means individual items, packs, or another unit.
Use separate lines for items that have different descriptions or unit prices. Compare the completed form with the actual items before release. For stock records, use the store inventory sheet template separately; this Word form does not adjust stock quantities.
Calculate each line amount from the unit price and quantity, then add the line amounts to obtain a subtotal. State the currency consistently. Record shipping charges or adjustments separately if applicable instead of hiding them inside an unrelated amount.
The template displays “Service Tax (10%)” as sample text. It is not an instruction to charge 10%. Replace or remove it according to the applicable transaction requirements, and check both the tax basis and the final total.
Remove the placeholder Latin text and insert wording appropriate to your order process. Review the statement “We accept all Debit and Credit Cards” rather than leaving it in place if it is inaccurate.
Describe relevant arrangements clearly and avoid using the form to collect full card details. Check that any signature area identifies what is being authorized rather than leaving its purpose unclear.
Check the order reference, address, descriptions, quantities, amounts, and instructions against the source records. Review print preview and save a final copy before sending it to the people handling the order.
The following hypothetical example uses one currency and excludes shipping, tax, and other adjustments.
| Description | Unit price | Quantity | Line total |
|---|---|---|---|
| Storage box, medium | 8 | 4 | 32 |
| Packing tape, roll | 3 | 2 | 6 |
| Subtotal | — | — | 38 |
If additional charges apply, show their descriptions and amounts before the final total. Confirm that the quantity unit matches the price unit.
Use this form according to the role you define in your workflow. A shipping order records the shipment-related order details you enter; it does not automatically replace every document needed for transport or billing.
This generic Word form is not a shipping label, customs declaration, or bill of lading. Confirm any required carrier or shipment-specific paperwork separately.
Review the full layout. The document contains repeated text and positioned elements. Check all visible copies of headings and sample values after editing.
Keep continuation pages clear. If more items require another page, repeat the order reference and identify which page contains the final total.
Test printing or PDF export. Check addresses, item rows, totals, and signatures for clipping or movement. Retain the editable Word file for future orders.
