These free field trip permission slip templates for Word help schools and childcare programs explain a visit and collect a parent or guardian’s response. Compare six editable formats, from a short single-trip slip to a multi-trip attendance form and a detailed parent letter.
Each ZIP contains a DOCX document. Choose a layout, replace the sample details and review it under your organization’s policies before sending it home. Several files contain dated examples or institution-specific provisions; the notes below identify changes needed before use.

Extract the ZIP and open the DOCX in Microsoft Word. Save a working copy and replace all placeholders, including repeated headings and consent text. These are editable documents, not an online consent service or a verified digital-signature workflow.
01. Contact and Support Information Permission Slip
Includes child details, parent contacts, an emergency contact, support needs, medical-history prompts, a conduct checkbox, participation wording and a parent signature/date.
Before sending home: Replace the fixed “age on July 8th” prompt with a relevant date or remove it. Add the destination, travel arrangements and exact times. The file combines participation, emergency-treatment and publicity language; review these separately and provide a clear, independent media choice where applicable.

02. School Activity Consent and Medical Update Form
Includes a child/class/activity/date consent statement, parent or carer signature, and a section for new medical information. It also refers to participation costs and medical assistance.
Before sending home: The source refers to the Department of Education and Training, One School, Medicare and an October 2012 enrollment threshold. It also makes insurance and medical-cost statements. These are not universal rules: replace them with current, approved wording for your school and jurisdiction. Supply the trip information the form says parents have read.

03. Short Single-Trip Permission Slip
Provides event name, date, beginning and ending times, child name, destination, accompanying person or group, transport method, parent contact, backup emergency contact and signature/date. The document has two repeated slips.
Before sending home: Add the learning purpose, organizer contact, return deadline, costs and what students should bring. If health or accessibility information is needed, identify the approved process for providing it rather than assuming this short slip collects it.

04. Childcare Multi-Trip Permission Table
Provides a childcare-program heading, several trip dates, destination details and a response table with child name, may-attend and may-not-attend trip numbers, parent signature and date.
Before sending home: Replace all sample 01-01-2024 dates. Number each trip and attach an unambiguous destination, date, departure time and return time to it. Clarify the sample arrival/departure headings. Protect each family’s response rather than circulating a completed table containing other children’s details.

05. Trip Consent with Medication Information
Includes child name, destination, date, purpose, parent and emergency phone numbers, a medication name/dosage/time table, behavior-policy wording and a parent/guardian signature.
Before sending home: Medication handling, packaging, authorization and information-sharing clauses need review under your organization’s procedures. A medication entry is not a substitute for a current, approved administration plan. Add departure/return times, transport, trip contact and return deadline.

06. Parent Letter and Detailed Trip Consent
Combines a sample educational visit letter with student and family details, emergency contacts, destination, date, curriculum link, school-bus travel, departure/return times, cost and a separate media-consent signature.
Before sending home: The letter describes an aquarium, but the consent paragraph names a fire station and an old July 2023 date. Correct every occurrence so parents authorize the same visit described in the letter. Replace the sample group-size and pizza-lunch funding statements with verified arrangements. Keep media permission separate from attendance consent.

What should a field trip permission slip include?
A parent should be able to understand the actual visit before responding. Add any missing information to your chosen form or an attached letter that is clearly identified on it.
- Student identification: Name, class or group, with enough detail for staff to match the response correctly.
- Destination and activity: Venue, location, educational purpose and the activities planned.
- Dates and times: Departure, expected return and any arrangements outside normal hours.
- Transport and supervision: How students will travel, who leads the trip and the relevant supervision arrangements.
- Costs and preparation: Amount requested, payment process, food, clothing, equipment and any support contact.
- Contact arrangements: A school or trip contact, a reachable parent/guardian and an alternate emergency contact.
- Support-information process: How to provide current allergy, health, accessibility or other participation information securely.
- Clear response: The specific visit being authorized, parent/guardian name, signature or approved response method, and date.
- Return instructions: Deadline, recipient and what happens if details or arrangements change.
Use a classroom management plan to help communicate relevant behavior expectations, adapting them to the visit. Keep the permission slip concise rather than copying an entire policy into the signature section.
Example: a clear school visit information block
The following is a fictional example to show how trip details can be presented. It is not a complete consent or emergency-treatment form.
| Detail | Sample wording |
|---|---|
| Visit | Year 5 visit to Example Science Museum, 25 Learning Road, Example City. |
| Date and times | 12 November 2026; depart school at 9:00 a.m.; expected return at 2:30 p.m. |
| Purpose | Explore the energy exhibit and complete a supervised observation worksheet. |
| Transport | School-arranged coach; staff will provide the confirmed meeting and collection instructions. |
| Preparation | Bring a packed lunch, water bottle and weather-appropriate clothing. |
| Cost | Enter the approved amount and payment instructions, including who to contact about support. |
| Response | Return the completed response to the class teacher by 5 November 2026. Contact the school office with questions or updated support information. |
Repeat the same destination and date in the actual consent statement. If an attachment explains the activities or arrangements, give it a title and version/date so the signed response can be connected to the information supplied.
Permission is one part of trip planning
A signed slip does not replace trip approval, suitable supervision, risk planning or an emergency response process. Nor should a generic signature be presented as automatically releasing a school from responsibility.
For example, England’s Department for Education provides educational visit guidance that distinguishes different types of visits and consent arrangements and discusses emergency planning. Follow the requirements and policies that apply to your own setting rather than treating one country’s guidance as a universal rule.
Organizers can use an action plan template to assign booking, communication and return-check tasks to named staff. If a particular hazard worksheet suits the activity, compare the risk analysis templates, while using the school’s approved assessment process for the actual visit.
Check returned forms before departure
- Match each response to the correct student and trip. Record missing, unclear or declined responses separately.
- Confirm that contact details are usable during the visit, not merely copied from an old record.
- Have authorized staff review relevant support information and arrange any necessary follow-up.
- Check that separate choices, such as media consent, are recorded accurately.
- Keep the necessary information available to appropriate trip staff and store completed forms through the school’s approved process.
- Communicate material changes and obtain any revised response required under the applicable policy.
Frequently asked questions
Are these permission slips available in Word or PDF?
The downloads contain Word DOCX files inside ZIP archives. Edit a document, check its layout and export a PDF if needed. A PDF export does not automatically become an electronically fillable form.
Which template is easiest for a short day trip?
Template 03 has a concise single-trip structure, including transport and emergency contacts. Add the purpose, deadline, costs and other information required by your school before using it.
Can one form cover several trips?
Template 04 provides a multi-trip table. Each trip and the parent’s response must be unambiguous. Whether a combined or standing consent is appropriate depends on your setting, the activities and applicable requirements.
Do not assume so. Some files contain medical wording, but it needs review under the school’s approved health and emergency procedures. Arrange required individual care and medication documentation separately where needed.
Should photo permission be part of the same signature?
Keep the media choice clear and distinct from attendance permission. Explain the intended use and follow the school’s privacy and consent procedures rather than assuming agreement to the trip also authorizes publicity.
Can parents respond digitally?
Use the school’s approved system if digital responses are accepted. These Word files do not verify signer identity, retain an audit trail or manage consent automatically.
